The mark consists of a rectangle and inside the top portion of the rectangle is a ribbon displaying the words "NATIONALLY RECOGNIZED" in block letters. Below and to the left side of the ribbon is a square, of which the bottom portion is shaded, and within the unshaded portion of the square is the block letter "A". To the right of the square are the words "LEAPFROG HOSPITAL SAFETY GRADE" in block letters.
