They work under limited MD supervision as their supervisors or team leaders. Their services in healthcare would be in primary care services for internal medicine, family practice medicine, and other subspecialties depending on the facilities they are assigned to provide the healthcare services. PhAs also continue to participate in designed CME programs, continuous medical education programs, which are designed to meet their service quality and higher goals in medical career development. PhA can work as PhA all their lives in narrow directions of clinical skill set and expertise based on their medical services they provide on every day job or PhA can strive for higher career promotion in a slightly harder pathway to reach the AML system to be finalized as licensed MDs as described below. - Attainable Medical Licensing for MDs, aka MD-AML: PhAs who strive to continue to be licensed as MDs in the AML system would try to meet the requirements of serving as Physician Associates for three to four years in medical facilities, passing the last USMLE step 3 composed of two components (clinical medicine practice and independent practices) and/or they have fulfilled one or as many as possible rotations of medical services in minimum four main divisions such as internal medicine, pediatrics, OBGYN and surgery, each with some minimum period of time; then, they will be promoted into a one-year residency program by the AML system. The AML system will be designed to add on more essential academic basic knowledge, clinical skills and clinical experiences besides all what they have acquired during their PhA healthcare services to reach the level of MD expertises. Then, upon finished this one-year residency program, they are issued licenses as MDs in primary care or restricted specialties depending on reviewing the periods of what services they have provided and so their according expertise level of clinical knowledge, experience skill sets. Once they are licensed as MDs, they can continue as the traditional pathways of MD services in our current system. The PhA services have never existed in our current system. PhAs who work longer than three to four years as PhA, but still does not meet these rotations are not eligible to be promoted into the AML; however, once they can fulfill those, they are again eligible to move on to the MD-AML. - This new system will strive to design working environment in all aspects of healthcare needs for PhAs to fully participate their services as mid-level providers as well as to meet their personal goals of medical career development
